The experienced staff of Oxford Bariatric understands that the insurance approval process can be a challenging experience. Insurance coverage for weight loss surgery varies from state to state by insurance carrier and employer group. Because every insurance policy is unique, it is important for you to thoroughly understand your Coverage and to know exactly what is and what is not covered through your plan.

Most insurance companies that cover weight loss surgery have specific criteria that must be met in order for a patient to qualify. By providing our office with specific information about your policy, we can assist you in determining if your policy covers weight loss surgery, as well as establishing the required criteria. We will need all of the insurance information from your insurance card. In order for us to serve you better, you will need to have this card with you when you contact our office.  Once it is established that your insurance policy covers weight loss surgery and the criteria is met, a letter of medical necessity to your insurance company may be required to obtain prior approval for surgery. As a service to you, we will be glad to write the letter to your insurance company.
